Separate the first bill from later bills

Write each charge in its labelled box. If one price covers both the visit and medicine, put that price in the “Visit or membership” box. Write “included” in the “Medicine” box so the price is counted once.

Leave the test box blank if you do not know that charge. A subtotal adds only the known charges; it is not the whole bill.

Ask about the terms too

The useful quote says what is included, when another charge arrives and how to cancel. A refund question matters most before a payment.

  • Is this a monthly charge or a longer commitment?
  • What happens when the visit ends without a prescription?
  • What is the cost after any opening offer?

Your cost-question worksheet

Enter the dollar amounts from your quote in the labelled boxes. If one price covers the visit and medicine, enter it in “Visit or membership” and write “included” in “Medicine”. Leave unknown charges blank. Print the notes you want to keep; these entries are not sent anywhere.
